This document is a page from a book of birth records in a column/table format from Radom, Poland. Unlike many of the records you might find in a metrical book of births, no new births are recorded on these pages. Instead, this record is primarily the baptismal record of a 28-year-old convert from Catholicism to Orthodoxy. Accordingly, the information and vocabulary differ slightly from what you would find on an average birth record.
